Alteas Health is seeking a skilled Part-Time Nurse Practitioner/Physician Assistant to join our team in providing high-quality pulmonary care to seniors in our care facilities. As a key member of our team, you will work part-time, two days per week, in a centralized region, analyzing and interpreting patients' histories, symptoms, and diagnostic information to develop appropriate diagnoses and treatment plans.
Key Responsibilities:
- Analyze and interpret patients' histories, symptoms, and diagnostic information to develop appropriate diagnoses and treatment plans.
- Diagnose or treat acute health care problems such as illnesses, infections, or injuries.
- Prescribe medications and maintain complete and detailed records of patients' health care plans and prognoses.
- Educate patients about self-management of acute or chronic illnesses and engage and coordinate with necessary internal and external clinical staff for continuity of care and best practice care coordination.
Requirements:
- Current and valid NP/PA licensure in IL
- Reliable transportation
- At least 6 months of NP/PA experience
Preferred Qualifications:
- 1+ years of Nursing Home experience
- Specialization in Pulmonary
