About the Role

Rochester Regional Health is seeking a highly skilled Clinical Specialist Wound Care- Home Care to join our team. As a key member of our Home Care department, you will be responsible for developing, implementing, and evaluating wound and ostomy specialty programs, as well as educating staff and patients on best practices.

Key Responsibilities
  • Program Development and Implementation: Collaborate with Quality Management staff to design, monitor, and participate in the continuous quality improvement process, and risk management components of our clinical program(s).
  • Assessment and Consultation: Perform direct/indirect care and documentation for select clients and caregivers that require specialist expertise, and provide consultation to PHV who are participating in the delivery of care and managing wound care and ostomy patients.
  • Education and Training: Convene with specialty team meetings on a regular basis to ensure that current and up-to-date information is disseminated to staff, and educate PHV as needed.
Requirements
  • ASN degree required, BSN degree preferred
  • WOCN and Home Care experience preferred
  • Current RN licensure in the State of New York
  • Wound Care Certification required, WOCNC preferred
  • Valid NYS driver's license required
