Outreach Services Associate

Job Summary
Northwell Health is seeking an Outreach Services Associate to join our team. The successful candidate will be responsible for scheduling patient/client appointments, maintaining files and records, and collecting statistical reports.
Key Responsibilities
- Contacts patients to schedule appointments for consultations, agency meetings, and diagnostic tests.
- Collects and organizes data required for statistical reports and completes reports as needed.
- Attends and participates in meetings as appropriate.
- Sets up files and maintains records.
- Provides incoming and in-process orientation.
- May be required to provide bilingual translation and/or guidance based on position.
- Provides basic education on HIV and STI prevention (training will be provided).
- Works outdoors for face-to-face community engagement.
- Drives company vehicle as needed.
Requirements
- High School Diploma or equivalent required.
- 0-1 years of relevant experience required.
- Fluency in English and Spanish highly preferred.
- Prior experience working with the LGBTQIA+ and MSM population highly preferred.
- Strong communication skills.
- Ability to work some nights and weekends.
- Valid NYS Driver's License.
