Key Responsibilities
  • Schedule Appointments and Coordinate Care

Work closely with patients, healthcare providers, and other stakeholders to schedule appointments and coordinate care. Ensure that patients receive timely and effective care, and that their needs are met.

Provide Patient Support and Education

Offer patients support and education to help them manage their health conditions, understand their treatment options, and make informed decisions about their care. Provide patients with information and resources to help them navigate the healthcare system.

Collaborate with Healthcare Teams

Work collaboratively with healthcare teams, including physicians, nurses, and other healthcare professionals, to ensure that patients receive comprehensive and coordinated care.

Identify and Address Health Disparities

Identify and address health disparities and barriers to care, and work to eliminate health inequities in our community.

Develop and Implement Care Plans

Develop and implement care plans that are tailored to the unique needs of each patient. Work with patients to set goals and develop strategies to achieve them.

Monitor and Evaluate Patient Outcomes

Monitor and evaluate patient outcomes, and make adjustments to care plans as needed to ensure that patients receive the best possible care.

Requirements
  • High School Diploma or Equivalent

A high school diploma or equivalent is required. A bachelor's degree in a related field, such as healthcare or social work, is preferred.

Excellent Communication and Interpersonal Skills

Excellent communication and interpersonal skills are required to work effectively with patients, healthcare providers, and other stakeholders.

Ability to Work in a Fast-Paced Environment

The ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities is essential.

Knowledge of Healthcare Systems and Policies

A knowledge of healthcare systems and policies, including Medicaid and Medicare, is preferred.

Ability to Speak Multiple Languages

The ability to speak multiple languages, including Spanish, is preferred.
