Job Summary

We are seeking a skilled and compassionate RN/LVN to join our nursing team at Skilled Nursing Professionals. As a key member of our care team, you will be responsible for providing high-quality patient care and support to our residents.

Essential Duties and Responsibilities
  • Confer with the Medical Director and attending physician regarding resident assignments and care plans.
  • Consult with physicians and nursing staff to develop and implement individualized care plans.
  • Examine residents and review medical records to identify potential health issues and develop strategies for prevention and intervention.
  • Initiate requests for consultation or referral and respond to resident, physician, and nursing staff requests.
  • Perform administrative duties, including completing medical forms, reports, evaluations, and charting.
  • Implement and maintain established policies, procedures, and quality assurance programs.
  • Make written and oral reports to the attending physician, Medical Director, and Administrator regarding resident status and care.
  • Participate in planning and evaluation of nursing service department policies and procedures.
  • Participate in facility surveys and resolve complaints and grievances.
  • Supervise and assist in management of the infection control program.
  • Assist in development of preliminary and comprehensive assessments of nursing needs.
  • Observe medication passes and treatments to ensure quality.
  • Administer services within the scope of nursing practice, including catheterization, tube feedings, and wound care.
  • Prepare and administer medications as ordered by the physician.
  • Review medication cards for completeness and accuracy.
  • Chart nurses' notes and reports of accidents/incidents.
  • Chart all changes in resident condition and response to treatment.
  • Perform routine charting duties and maintain adequate stock levels of medications and medical supplies.
Qualifications
  • High School Diploma or equivalent.
  • Graduate of an accredited Licensed Vocational or Practical Nurse program.
  • Active license to practice as a Licensed Vocational Nurse or Licensed Practical Nurse in this state.
  • Ability to read, write, speak, and understand the English language.
  • Ability to make independent decisions and deal tactfully with personnel, residents, and visitors.
  • Knowledge of nursing and medical practices, laws, regulations, and guidelines.
  • Leadership and supervisory ability.
Work Environment

This position requires working throughout the nursing service area, moving intermittently during working hours, and interacting with residents, personnel, and visitors under all conditions and circumstances.

The successful candidate will be subject to frequent interruptions, hostile and emotionally upset residents, and exposure to infectious waste, diseases, and hazardous chemicals.

The physical and sensory requirements of this position include moving intermittently throughout the workday, coping with the mental, emotional, and stress of the position, and meeting the general health requirements set forth by the policies of this facility.
