Job Summary:

South Shore Health is seeking a highly skilled Physician Assistant or Nurse Practitioner to join our Hospital Medicine team. As a key member of our team, you will be responsible for evaluating, diagnosing, and treating patients in a fast-paced hospital setting.

Responsibilities:

• Obtain a detailed and accurate medical history and perform comprehensive physical exams for patients admitted to the hospital.
• Order necessary diagnostic tests, consultations, and procedures as indicated or requested by the Hospital Medicine Consultant or attending physician.
• Identify and manage chronic medical conditions and provide continuous care to hospitalized inpatients.
• Prescribe medications as necessary and write orders in accordance with current practice guidelines.
• Write complete and comprehensive notes on all patients using appropriate electronic systems consistent with Hospital policy.
• Facilitate timely care and actively participate in South Shore Health's readmission reduction efforts.
• Collaborate with the patient care team to develop and facilitate care plans and proactively manage patients to prevent complications.
• Prepare patients for discharge, including patient and family discharge teaching, continuing care, and rehabilitation arrangements in conjunction with Nursing and Care Progression.

Requirements:

• Completion of Advanced Physician Assistant or Nurse Practitioner training program from an accredited institution.
• All licensure re-certifications, as required.
• Demonstrated history of proficiency in the delivery of medical care to adult patients.
• Successful completion of the SSH Credentialing process.
• ACLS Certification.
• 2 years of Acute Medicine experience, preferred.
