Hospice Clinical Manager Position

HopeHealth is seeking an experienced Patient Care Manager to lead its Massachusetts hospice team. The ideal candidate will have a strong background in nursing and experience in a Home Care/Hospice setting.
Key Responsibilities:- Develop, administer, and oversee all aspects of the team to ensure high-quality services to patients, families, and caregivers.
- Monitor and oversee the efficient and effective use of human, fiscal, and material resources.
- Ensure nursing care and overall patient care are coordinated, safe, competent, and effective.
- Guide and support team staff in their professional development and ensure they remain current on required competencies.
- Represent the organization in the community and foster effective communication with patients, families, and caregivers.
- Registered Nurse with a Bachelor's degree in Nursing.
- Three to five years of nursing experience, with a minimum of two years in a Home Care/Hospice setting.
- Direct clinical supervisory experience required.
- Ability to achieve HPNA certification within one year of employment.
