Social Work Specialist

We are seeking a skilled Social Work Home Health Social Worker to join our team at LHC Group in Easton, Massachusetts. As a Social Worker, you will play a vital role in assisting patients and families in coping with problems resulting from severe or long-term illness, and with difficulties in recovery and rehabilitation.
The ideal candidate will have a Master's Degree from a school of Social work accredited by the Council of Social Work Educations, and one year of social work experience in a health care setting. A current CPR certification, current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation are also required.
As a Social Worker at LHC Group, you will have the opportunity to work with a team of dedicated professionals who are passionate about delivering high-quality, cost-effective care that supports our patients when and where they need it. Responsibilities of the Social Worker include assessing and treating social and emotional factors related to patient's illness to determine ability to cope with daily living problems, instructing health care team members on community resources available to assist patients, and planning for continuity of care with hospitals and community agencies.
