About the Role

As a Per Diem Case Management RN, you will play a vital role in coordinating patient care and developing comprehensive plans to achieve optimal rehabilitation results. Your expertise will be instrumental in facilitating effective communication between patients, families, and interdisciplinary teams. Key responsibilities include:

  • Completing departmental orientation and initial competencies
  • Collaborating with other departmental staff to deliver patient-centered care, identify barriers to care and or discharge, and develop solutions/resolutions
  • Documenting progress and outcomes per workflow timeline and content requirements, including completion of the Individual Plan of Care (IPoC) per CMS guidelines

We are seeking a skilled and compassionate individual with a passion for delivering high-quality care. A background in social work or case management is preferred, with certification in Case Management or Rehabilitation Nursing a definite asset. Effective oral and written communication skills, as well as proficiency in basic computer applications, are essential for success in this role.
