About the Role

VNS Health is seeking a skilled Substance Use Counselor to join our team in Queens, New York. As a key member of our Behavioral Health team, you will play a vital role in providing compassionate care and support to individuals struggling with substance use disorders.

Key Responsibilities
  • Conduct phone and field outreach to engage with clients and their families, building trust and relationships to facilitate their recovery journey.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans, ensuring clients receive comprehensive support and services.
  • Provide education and guidance on recovery-based services, symptom management, and coping skills, empowering clients to take an active role in their recovery.
  • Advocate for clients' needs and interests, connecting them with community-based resources and services to support their well-being.
  • Document client interactions and progress in electronic health records, ensuring accurate and timely reporting.
Requirements
  • High school diploma or equivalent required; New York Certified Recovery Peer Advocate (CRPA) certification preferred.
  • Minimum one year of experience in a mental health, substance use treatment program, healthcare, or human services setting, with a focus on working with severely mentally ill, psychologically traumatized, and/or substance-using populations.
  • Effective communication and interpersonal skills, with the ability to work independently and collaboratively as part of an interdisciplinary team.
  • Computer literacy, including electronic health records, word processing, email, internet research, and data entry.
  • Valid New York State driver's license, as determined by operational/regional needs.
