Job Title: Licensed Practical Nurse - Full Time - Home Health

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team in Danville, Pennsylvania. As a member of our Home Health team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role:

The Licensed Practical Nurse (LPN) in Home Health is responsible for providing high-quality patient care and performing technical skilled care in the patient's home. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities:
  • Provide quality patient care and perform technical skilled care in the patient's home
  • Develop and implement individualized care plans in collaboration with the healthcare team
  • Monitor and report patient progress to the supervising nurse and/or physician
  • Provide education and support to patients and their families on disease management and self-care
  • Collaborate with other healthcare professionals to ensure seamless care coordination
Requirements:
  • Current LPN licensure in the state of practice
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
  • CPR Certification required
