Responsibilities- Psychosocial and Bereavement Services: Provide compassionate support to patients with a life expectancy of seven days or less, focusing on their immediate care needs, including funeral arrangements and advance directives.
- Hospice Informationals and Consent Process: Respond to referrals, conduct informational sessions, and lead the consent process for new patients, ensuring they understand our services and make informed decisions.
- Charting and Visits: Complete all necessary charting and visits in a timely manner, adhering to state and federal regulations, as well as agency policies and procedures.
- Care Planning and Case Conferences: Participate in care planning and case conferences, ensuring the involvement of patients and their families in assessing their needs and developing personalized care plans.
- Community Referrals: Provide external referrals to community supports, facilities, and resources as requested by patients, families, and community partners.
- STAR Mentality: Demonstrate a selfless, tough, accountable, and respectful approach to your work.
- Education and Experience: Possess a Bachelor's degree from an accredited Social Work program and at least two years of experience in Social Work, Case Management, or a related field. Additionally, you should have at least one year of experience in a healthcare setting and preferably in hospice or home care.
