Job Summary

Key Responsibilities
  • Provide professional nursing services under the direction of a RN in compliance with state and federal regulations, applicable licensure/certification requirements, healthcare standards, and CenterWell Home Health policies and procedures.
  • Develop and implement individualized plans of care to meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address these needs.
  • Provide skilled nursing interventions, including wound care, medication management, and education on disease management and prevention.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements
  • Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
