Job Summary
We are seeking an experienced Primary Care Provider to join our team at Dignity Health Medical Group - Sequoia in Belmont, CA. As a Primary Care Provider, you will be responsible for providing high-quality patient care, conducting comprehensive health assessments, and developing and implementing patient care plans.
Key Responsibilities
- Conduct comprehensive health assessments and diagnose and treat acute and chronic illnesses.
- Develop and implement patient care plans, prescribe medications and therapies, and provide health promotion and disease prevention education.
- Collaborate with other healthcare professionals, consult with physicians and specialists as needed, and coordinate patient care across various settings.
Requirements
- At least 2 years of experience as a Nurse Practitioner or Physician Assistant.
- A valid and unrestricted license to practice in the State of California.
- Nationally recognized certification (as determined by Medicare).
- Completion of state mandated educational courses.
Compensation and Benefits
- Competitive salary range of $120,000 - $140,000/year.
