Position Overview: As an Advanced Practice Provider (APP), you will be responsible for overseeing the care of patients with both acute and chronic health conditions. Your role will encompass preventive and primary care services delivered in a dedicated clinic setting, utilizing telehealth, remote patient monitoring, and mobile health units to enhance patient access and care quality.

Work Schedule: This position requires a commitment of 40 hours per week, with a standard work schedule from Monday to Friday, 8:00 AM to 4:30 PM.

Patient Demographics: The focus will be on outpatient care, primarily serving adult patients, with the capability to provide limited pediatric care.

Qualifications:

  • Educational Background: Completion of an accredited Nurse Practitioner Program and current licensure or eligibility for licensure as an Advanced Practice Registered Nurse (APRN).
  • Experience: A Master's degree or higher in Nursing, specifically as a Family Nurse Practitioner, along with a minimum of 2 years of clinical experience in advanced practice nursing.
  • Preferred Qualifications: A Doctorate in Nursing Practice and advanced training in Diabetes Management, with a focus on chronic disease management including diabetes, hypertension, obesity, and chronic kidney disease.

Supervision: The APP will work under the guidance of a Primary Supervising/Collaborating Physician employed by the Medical University of South Carolina Health.

Licensure and Certifications:

  • All certifications must be current prior to the start date, including Basic Life Support (BLS).
  • Applications for DHEC and DEA licenses must be initiated before the first day of employment.
  • Certification as a Diabetes Educator (CDCES) or significant experience in diabetes management, with a commitment to obtain certification within 12-18 months of hire.

Key Responsibilities:

  • Conduct comprehensive patient assessments through detailed history-taking and physical examinations.
  • Evaluate and manage patient care based on clinical findings and diagnostic test results, adhering to established practice agreements.
  • Perform therapeutic procedures as permitted by procedural privileges and in accordance with the supervising physician's approval.
  • Facilitate referrals to other healthcare professionals and consult with the collaborating physician as necessary.
  • Document patient interactions and ensure accurate billing for services rendered.
  • Utilize evidence-based practices and current research to inform clinical decision-making.
  • Engage in quality improvement initiatives and support clinical research efforts.
  • Participate in compliance and regulatory activities to uphold organizational standards.
  • Contribute to the education of students from various health professions, including advanced practice provider students.
  • Maintain all required licenses, certifications, and continuing education as mandated by state laws and organizational policies.

Physical Demands:

This role requires the ability to perform job functions while standing, walking, and sitting. Candidates must be capable of bending, kneeling, climbing stairs, and reaching in all directions. Physical qualifications include the ability to lift and carry objects weighing up to 50 lbs unassisted and to perform repetitive motions with hands and arms.
