Join our team at Sunrise Services Inc. as a Behavioral Health Clinician and play a vital role in the Geriatric Transitions Program (GTP). This unique 90-day program aims to stabilize community placements and reduce hospitalizations for clients 60 years or older with serious mental illness or dementia. As a clinician, you will work intensively with clients, their support networks, and the environments they live in to ensure they receive the behavioral health support they need to remain in their homes and placements.
The GTP caseload is low, with a maximum of 10-12 clients, allowing for personalized support and flexible scheduling. You will work a hybrid-remote model, seeing clients in the community while completing office hours remotely. This position is ideal for professionals seeking a challenging and rewarding role in mental health.
Key Responsibilities:
- Assess client needs, risk factors, and barriers to community placement at referral.
- Provide interim brief therapies, counseling, and psychoeducation for clients.
- Develop positive behavioral support plans and provide mental health education and training for client families and caregivers.
- Crisis stabilization and care coordination.
Requirements:
- Master's degree in social work, counseling, or a related field.
- At least two years of experience in supervised direct treatment of persons with mental illness and/or developmental disabilities.
- Mental Health Professional (MHP) licensure or equivalent.
