Crossroads Hospice & Palliative Care is seeking dedicated Nurse Practitioners to join our team in delivering exceptional end-of-life care. As a palliative care specialist, you will play a vital role in improving the quality of life for our patients and their families.

We offer a competitive annual salary of $115,000, along with a comprehensive benefits package that includes health, dental, vision, 401(k), PTO, and continuing education reimbursement.

The ideal candidate should possess at least 2 years of acute care experience, a Master's degree in Nursing, and an active unencumbered state NP license. Additionally, they must have a DEA number and National Provider Identifier (NPI).

This community-based role involves traveling between patient visits and conducting detailed assessments, recertification for hospice patients, and providing high-quality care. Our team prioritizes work-life balance and offers ongoing training and support to ensure your success.
