Schedule and Salary InformationThis role offers a competitive salary: $85,000 per year, commensurate with experience. The schedule is Monday to Friday, 8:30 AM - 5:00 PM.
Job SummaryWe seek an experienced Nurse Practitioner to join our team. As a key member of our interdisciplinary team, you will provide high-quality patient care and work collaboratively with other healthcare professionals to achieve the best possible outcomes.
Required Skills and Qualifications- A minimum of 2 years of Nurse Practitioner experience in an acute care environment.
- Home Health, Palliative Care, or Hospice Experience is Required.
- Masters of Science in Nursing.
- Active unencumbered state NP license.
- Current driver's license and auto liability coverage.
- Detailed assessment skills as part of this role require recertification for Hospice patients.
- Interest in a community-based role that requires travel between patient visits.
- Drug Enforcement Agency (DEA) number Required.
- National Provider Identifier (NPI) Required.
