We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Omaha NE HH. Responsibilities:

  • Provide nursing visits to patients in their homes, as prescribed by physicians and in accordance with treatment plans.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to address these needs.
  • Administer medications, perform wound care, and provide other treatments as ordered by physicians.
  • Document patient interactions, treatments, and outcomes in a timely and accurate manner.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive patient care.

Requirements:

  • One year of clinical experience and one year of home health experience.
  • Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records.
