We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities:
  • Provide professional nursing services under the direction of a RN, ensuring compliance with state regulations and CenterWell Home Health policies.
  • Develop and implement individualized care plans, focusing on patient-centered care and promoting independence.
  • Collaborate with healthcare teams to ensure seamless care coordination and effective communication.
  • Conduct thorough assessments, identify patient needs, and develop strategies to address them.
  • Administer medications, treatments, and therapies as prescribed by physicians, while maintaining accurate records.
  • Provide education and support to patients and their families, promoting health literacy and self-management skills.
  • Participate in quality improvement initiatives, contributing to the development of best practices and evidence-based care.
Requirements:
  • Graduate of an accredited LPN program or equivalent.
  • Current LPN licensure in the practicing state.
  • Valid driver's license, reliable transportation, and auto insurance.
  • Current CPR certification.
  • At least two years of experience as an LPN in a clinical setting, preferably in home health or hospice care.
