- Facilitate effective communication between physicians and patients.
- Utilize knowledge of community resources to assist patients in accessing necessary services.
- Provide information on financial assistance options, including Medicare and Medicaid.
- Act as an advocate for patients and their families, ensuring their needs are met.
- Offer emotional support to those facing crises or serious health diagnoses.
- Connect patients with local support groups and resources.
Maintain thorough and accurate documentation in medical records regarding interactions with patients, families, and healthcare providers. Ensure compliance with state and federal regulations while counseling patients about available benefits.
Care CoordinationAssist in developing care plans for patients identified through case management and support the team in addressing age-specific psychological needs. Help coordinate transfer arrangements for patients as needed.
CollaborationWork collaboratively with skilled nursing facilities, rehabilitation centers, and home health agencies to ensure seamless care transitions.
QualificationsEducation: A Bachelor’s degree in Social Work is required.
Experience: A minimum of one year of experience in a related field is required, with preference given to candidates with hospital or long-term care experience.
Skills: Strong critical thinking skills, decisive judgment, and the ability to work independently in a fast-paced environment are essential.
