Physical Therapist

We are seeking a skilled Physical Therapist to join our team in providing high-quality homecare services to patients with disabilities, injuries, and/or diseases.
Key Responsibilities- Evaluate and treat patients using physical, chemical, and mechanical means in accordance with physician prescriptions and referrals.
- Administer direct patient care, utilizing appropriate evaluation techniques and designing treatment programs to achieve established goals.
- Monitor patient tolerance and status during treatment, modifying programs as needed, and responding to emergency procedures when necessary.
- Submit required documentation and participate in quality improvement chart reviews and activities.
- Interact with interdisciplinary staff and attend family meetings/conferences to ensure patient welfare and management.
- Master's Degree in Physical Therapy or equivalent combination of education and experience.
- Current License to practice as a Physical Therapist in New York State, plus specialized certifications as needed.
- 0-1 years of relevant experience.
