BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Bay Village, Ohio.

We are looking for an experienced Physical Therapist to provide high-quality care to our adult and geriatric clients in their homes throughout Northeast Ohio. As a member of our multidisciplinary health care team, you will work closely with nurses and other healthcare professionals to develop and implement individualized care plans.

Responsibilities include:

  • Conducting home visits to assess clients' needs and develop personalized care plans.
  • Performing diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
  • Developing and implementing manual therapeutic exercises, gait training, and other interventions to improve clients' functional abilities.
  • Collaborating with nurses and other healthcare professionals to revise care plans and ensure seamless care coordination.
  • Educating clients and their families on rehabilitative care and activities to promote independent living.
  • Accurately documenting observations, interventions, and evaluations in our state-of-the-art electronic medical records system.

Qualifications include:

  • A current Ohio Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
