Job Title: Physical Therapist - Home Health

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Bay Village, Ohio.

About the Role:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Physical Therapist with BAYADA, you will have the opportunity to work with a diverse range of clients, from adults to geriatric patients, and provide individualized care plans to help them achieve their goals.

Responsibilities:
  • Conduct thorough assessments and develop personalized care plans to address clients' physical therapy needs.
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to help clients achieve optimal mobility and function.
  • Collaborate with nursing staff and other healthcare professionals to ensure comprehensive care and coordination.
  • Document client progress and adjust care plans as needed to ensure optimal outcomes.
Requirements:
  • Current Ohio Physical Therapist license.
  • Minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
