Job Summary:
The Post-Acute Liaison is a key member of our team, responsible for generating business from referrals sources to meet the community's revenue and census goals. This role involves developing relationships with community hospitals, physician offices, and other community outreach programs to drive business growth.
Key Responsibilities:
- Develop and maintain relationships with referral sources to drive business growth
- Coordinate external professional relationships to achieve revenue and census goals
- Work closely with the admission coordinator and other key team members to ensure efficient referrals and care transitions
- Engage with collaborative network leadership, risk holding physicians, and managed care partners to share insights and drive program enhancement opportunities
