Benefits
  • We offer a competitive salary range of $27.96/hour - $32.85/hour for Public Health Nurse I and $29.79/hour - $35.00/hour for Public Health Nurse II.

Responsibilities

The Public Health Nurse I or II will provide personalized case management to pregnant and postpartum individuals and their families. The role involves conducting home and community visits, evaluating and performing physical nursing assessments, creating care plans based on client needs, and educating clients and families on health conditions and public health topics.

  • Conduct thorough assessments of clients' physical and emotional needs, and develop tailored care plans to meet those needs.
  • Collaborate with healthcare providers, social services, and community agencies to ensure seamless care coordination and support.
  • Promote healthy behaviors and lifestyles among clients and their families, using evidence-based practices and cultural sensitivity.
  • Maintain accurate records of client interactions and progress in electronic health records, using mobile technology for remote work and home visits.
Requirements
  • An associate degree in Nursing from an accredited College of Nursing (Public Health Nurse I) or a bachelor's degree in Nursing (Public Health Nurse II).
  • At least 2 years of experience in nursing, preferably in OBGYN, postpartum, public health, case management, home visiting, or related disciplines.
  • Knowledge and proficiency in Electronic Health Record systems.
  • Current and unrestricted Ohio Registered Nurse license; Ohio Driver's License; BLS CPR Certification.
