Job Summary:
The Care Manager is accountable for the management and coordination of the patient's plan of care beginning with the admission, active phase of care, and addressing of discharged needs and concerns to ensure the patient receives appropriate, cost-effective, quality care during their in-patient stay.
Key Responsibilities:
- Develop and implement patient-centered care plans to meet individual needs.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Conduct thorough assessments to identify patient needs and develop effective discharge plans.
- Communicate with patients, families, and healthcare providers to ensure informed decision-making and patient satisfaction.
- Monitor and evaluate patient progress, making adjustments to care plans as needed.
Requirements:
- Graduate of an accredited Registered Nurse diploma or college program.
- Bachelor's Degree in Nursing preferred.
- At least 3-5 years clinical experience as a registered nurse in an acute care hospital required.
- 2 or more years of hospital case management, utilization reviews, or discharge planning experience preferred.
- Recent acute care hospital experience preferred.
- 1-2 years of critical care experience preferred.
Working Conditions:
- Personal Protective Equipment (PPE) worn.
- Extensive computer time.
- Walking the units.
