Job Summary:
Soliant is seeking an experienced Advanced Practice Pharmacist to join our team in an Oncology Ambulatory Clinic setting. As a key member of our team, you will be responsible for providing high-quality patient care and contributing to the development of our Oncology Clinic program.
Responsibilities:
- Provide direct patient care services, including medication management, lab testing, and patient education.
- Work closely with Oncology Physicians and Pharmacy Administration to build and implement our Oncology Clinic program.
- Develop and maintain collaborative practice agreements with healthcare providers.
- Conduct medication reconciliations, review planned therapies, and provide medication counseling.
- Order lab tests, medications, and refills under a collaborative practice agreement.
- Communicate with patients and healthcare providers to ensure seamless care transitions.
Requirements:
- Minimum 1+ years of pharmacy experience required.
- PGY2 Oncology or (PGY1 with BCOP) certification preferred.
- BCACP or BCOP or BCPS certification preferred.
- 1,500 hours of clinical experience under collaborative practice agreement preferred.
Compensation and Benefits:
- $210,000-$225,000 annually.
- $10,000 bonus.
