Case Manager

Home Start, Inc. is seeking a skilled Case Manager to join our team. As a Case Manager, you will provide case management services and support to short-term and transitional housing clients in our Rapid Rehousing program.
Key Responsibilities- Coordinate with referral sources and respond within 48 working hours;
- Provide mobile advocacy services and support to victims of crime;
- Meet with survivors and complete intake and assessment;
- Develop individualized plans for delivery of housing and supportive services that are tailored, survivor-driven, and trauma-informed;
- Help participants establish linkages to community services and resources that help them achieve their goals;
- Work with survivors in an advocacy capacity, helping them understand and navigate through various systems;
- Help survivors establish and strengthen their support network of peers, family, and community members;
- Encourage participants to make choices that reduce risk and promote safety;
- Assist qualified survivors complete paperwork for the California Victim Compensation Program;
- Complete and maintain client records that document services provided;
- Facilitate access to flexible financial assistance to address barriers to safety and stability;
- Provide follow-up services to RRH clients for at least 6 months after they leave the program;
- Complete all required training, including DV Counselor training and pre- and in-service training on topics such as Trauma-Informed Care and Client Confidentiality/HIPPA;
- Participate in system data collection and complete necessary paperwork and forms.
We are seeking a candidate with a Bachelor's Degree in human services or a related field, or equivalent work or volunteer experience and training. The ideal candidate will have excellent communication and computer skills, as well as the ability to work well with diverse populations. Bilingual (Spanish/English) speakers are preferred. The candidate must be a Certified DV Counselor or willing to complete the training within 3 months of hire. A valid California driver's license, safe driving record, automobile insurance, and a reliable vehicle are also required.
Work EnvironmentThe Case Manager will work in an office environment and travel to meet with clients and attend agency and community meetings.
