Job Title: Case Manager

At The Salvation Army USA Western Territory, we are seeking a skilled Case Manager to join our team. As a Case Manager, you will play a vital role in evaluating the ongoing needs of individuals and providing intensive case management services to aid residents in stabilizing their lives and minimizing future episodes of homelessness.

Key Responsibilities:
  • Evaluate the ongoing needs of individuals and develop case plans to include goals and objectives.
  • Make appropriate referrals to other agencies and services as necessary.
  • Provide follow-up to ensure that referrals are completed and track progress of residents.
  • Facilitate communication and coordination of services between care/service/support providers.
  • Promote resident self-advocacy and self-determination.
Requirements:
  • Bachelor's degree in a behavioral science-related field, such as social work, sociology, or psychology.
  • Minimum two years of experience working in a residential program for homeless families.
  • Minimum two years of case management experience.
Skills and Qualifications:
  • Ability to provide case management services, including resources, tools, and counseling to assist residents in achieving their case plan goals.
  • Detail-oriented with strong organizational and time management skills.
  • Strong oral and written English communication skills.
  • Strong listening, critical thinking, and problem-solving skills.
