In this critical role, the Intake Assessor serves as the first point of contact for patients and families entering our care at CommuniCare Family of Companies. With a focus on providing exceptional service, this professional ensures a smooth and efficient intake process, coordinating with clinical teams and payors to achieve timely admissions.

Responsibilities
  • Assist with admission/intake process for all patients
  • Coordinate with Clinical Liaisons and physicians
  • Verify insurances and obtain authorizations as necessary
  • Maintain and communicate accurate census records
