MDS Coordinator Job Description

We are seeking a skilled and experienced MDS Coordinator to join our team at Therapy Solutions. As an MDS Coordinator, you will play a critical role in ensuring the highest quality of care for our residents.

Responsibilities:
  • Review patient care plans to ensure they are up-to-date and reflective of the resident's current needs.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Conduct regular assessments to identify changes in the resident's condition and make recommendations for updates to the care plan.
  • Ensure that all personnel involved in providing care to the residents are aware of their care plans and are working together to achieve the goals outlined in the plan.
  • Monitor and guide the completion of PPS and OBRA assessments to ensure that they are accurate and complete.
  • Manage the Utilization Review (UR) process to ensure that it is conducted in a timely and efficient manner.
  • Oversee the maintenance of accurate and up-to-date medical records, including MARs, treatment flow sheets, and physician orders.
Requirements:
  • A nursing degree from an accredited college or university.
  • A current, unencumbered, active license to practice as a registered nurse in this state.
  • MDS Certification is preferred.
  • Knowledgeable and experienced in the RAI process, CAAs, RUG categories, and MDS 3.0.
  • Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
  • Ability to maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
