Director of Social Services

Job Summary:
The Director of Social Services at Tarrytown Rehabilitation & Nursing Center is responsible for providing case management to short-term and long-term residents and families with psycho-social needs/concerns. This role requires a strong understanding of social work principles and practices, as well as excellent communication and interpersonal skills.
Key Responsibilities:
- Coordinate the continuity of care for residents and ensure the safety and well-being of residents at Tarrytown Hall Care Center.
- Develop and implement care plans that meet the unique needs of each resident.
- Provide education to residents and their families regarding advance directives and other relevant topics.
- Coordinate the implementation of advance directives with the clinical team.
- Maintain accurate and timely medical record documentation.
- Assist with care plan meetings and ensure residents and their families are invited in accordance with facility policy.
- Coordinate all aspects of discharge planning to ensure a smooth transition back to the community or any other living environment.
- Make referrals to public or private service providers as needed.
- Complete MDS as per facility policy.
- Develop and coordinate policies and procedures for the Social Service Department.
- Record and track grievances/complaints received by residents and their families.
- Ensure grievances/complaints are followed through in a timely manner.
- Provide emotional support and counseling as needed to residents and their families.
- Assist in the implementation of behavior management plans.
- Work closely with psychiatry/psychology to ensure that residents' mental/emotional needs are being met and managed effectively.
- Coordinate palliative/hospice care, including referrals to outside agencies, education, and emotional support of residents and their families.
- Participate in quality assurance and improvement processes within the facility.
- Participate in inspections (surveys) made by authorized agencies.
- Demonstrate the mission, vision, and values of the facility through professional excellence.
- Maintain compliance with state and federal regulations.
- Perform all other duties as determined by the Administrator.
Requirements:
- Minimum of bachelor's degree in social work or a related field.
- Master's degree in social work preferred.
- Professional social work license (for facilities greater than 120 beds).
Preferred Qualifications:
- Experience in case management/care coordination of residents in short and long-term care.
- Knowledge of discharge planning, crisis intervention, MDS completion, community resources, entitlements, family/individual counseling.
Working Conditions:
The Director of Social Services will work in a fast-paced environment with frequent interruptions. The employee will be required to stand, walk, talk, and hear, and use hands and arms to handle items, lift, feel, and reach.
Interpersonal Relationships:
The Director of Social Services will communicate effectively with residents, families, visitors, co-workers, physicians, and other employees.
Essential Functions:
The Director of Social Services will adhere to the facility's policies and procedures, participate in screening of job applicants, hiring new employees, and recommending promotions, transfers, and dismissals.
Personnel Functions:
The Director of Social Services will be responsible for ensuring adequate staffing to perform the duties required of the social work department, while staying within the specified budget allocated to the department.
Staff Development Functions:
The Director of Social Services will conduct in-service training to new/existing social service employees to explain policies, work procedures, and to demonstrate use of programs used.
Resident Rights:
The Director of Social Services will maintain confidentiality of all pertinent resident care information and report all grievances/complaints made by the resident to the Administrator.
