As a key member of our mental health services team, you will be responsible for providing comprehensive biopsychosocial assessments, behavioral health services, and social work support to individuals, couples, and families.
Key Responsibilities- Provide age and culturally appropriate comprehensive biopsychosocial assessments for mental health, substance abuse, and medical needs in the primary care setting.
- Provide behavioral health services to individuals, couples, and families, including counseling, intake, assessment/evaluation, diagnosing, treatment planning, crisis intervention, and referrals to community agencies.
- Provide liaison services, social work support, and referrals to local resources and community agencies, including medication assistance programs.
- Collaborate with Quality Improvement teams to develop and implement processes for data collection.
- Master's degree in Social Work and three (3) years post licensure experience.
- Must maintain relevant licensure as a Licensed Clinical Social Worker (LCSW) in the state.
- Must possess knowledge of counseling techniques, crisis intervention, and be able to make independent judgments related to counseling as a direct service.
- Experience and skills in group facilitation and planning processes.
