Job SummaryWe are seeking a skilled Nurse Practitioner to join our team on a per diem basis. As a Nurse Practitioner with CCS, you will have the opportunity to make a meaningful difference in the lives of our members, while working in a dynamic and supportive environment.
Responsibilities- Conduct comprehensive assessments of assigned member populations, including timely completion and submission of required documentation.
- Ensure accurate and thorough documentation of pertinent medical conditions, in compliance with CMS/DHHS and client requirements.
- Support the physician/patient relationship, ensuring timely and adequate communication, and documentation of assessment findings and recommendations.
- Make general recommendations to members to improve their knowledge of chronic conditions, and address identified gaps in care.
- Recognize emergent or urgent situations, and take appropriate action as specified in Inovalon policies.
- Active un-encumbered license to practice nursing.
- Nurse Practitioner Certification from ANCC or AANP.
- Board Certified through AANP or ANCC.
- Maintains current CPR certification.
- Compliance with recommended Healthcare Personnel Requirements for vaccinations and preventive testing.
- Home Health experience.
- Understanding of Medicare, Medicaid, and Health Plan benefit structures.
- Ability to multitask, and excellent customer service skills.
