### Key Responsibilities

* Administer skilled nursing care to patients on an intermittent basis in their place of residence
* Develop and implement individualized plans of care in collaboration with patients, families, and healthcare professionals
* Conduct thorough assessments of patients' physical, emotional, and social needs
* Provide education and support to patients and families to promote health and wellness
* Collaborate with interdisciplinary teams to coordinate care and services

### Requirements

* Current licensure as a Registered Nurse in the state of Mississippi
* Experience in home health or a related field
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
