Job Overview

Hospice of Grace and Palliative Care, Inc. is seeking a skilled Terminal Illness Support Specialist to join our interdisciplinary team.

The estimated annual salary for this role is $90,000 - $115,000, commensurate with experience.

About the Job

This position involves working closely with patients and their families to provide emotional and practical support during a difficult time. The successful candidate will possess excellent listening and communication skills, as well as the ability to maintain confidentiality and work effectively in a fast-paced environment.

Key Responsibilities

  • Develop and implement individualized care plans that prioritize patient comfort, dignity, and quality of life.
  • Conduct regular assessments to identify changing needs and preferences, making adjustments to care plans as necessary.
  • Collaborate with healthcare professionals to coordinate patient care, ensuring seamless transitions between settings.
  • Provide education and support to patients and their families, addressing questions and concerns related to terminal illness and end-of-life care.
  • Maintain accurate and timely documentation of patient interactions, progress notes, and care plan updates.

Requirements

  • Bachelor of Science degree in nursing (BSN)
  • Current RN licensure and certification in hospice and palliative care (preferred)
  • Minimum 2 years experience in hospice care, with a focus on patient and family support
  • Strong knowledge of hospice eligibility criteria, Medicare and Medicaid regulations, and state-specific laws governing end-of-life care
