Position Overview

The Children's Residential Services Program is dedicated to supporting children and adolescents aged 5 to 18 who are facing significant emotional or behavioral challenges. These individuals often require more intensive treatment after unsuccessful attempts with less restrictive options. AltaPointe Health provides a nurturing and structured environment, offering tailored therapeutic services around the clock. Our program also extends support to families or caregivers, fostering attachment and connection during treatment and throughout the discharge process. We cater to children and adolescents with co-occurring mental health and substance use disorders through a specialized service track. Involvement from consumers, families, caregivers, educational personnel, and relevant stakeholders is integral to the treatment and discharge planning process. Our primary aim is to enhance each child's or adolescent's functioning level and facilitate a transition to a less restrictive care setting.

Key Responsibilities

As an LPN in a healthcare setting, you will be responsible for various essential tasks, including:

  • Conducting the admission process for consumers, which encompasses: Medical Checklist, Physical Health Assessment, Nutritional Screening, Medication List, Medication Administration Record, vital signs, height/weight measurements, transcribing physician orders, AIMS assessments, and admission documentation.
  • Monitoring consumer behavior during group activities, documenting observations in consumer records, and collaborating with the Treatment Team to ensure accurate information exchange during shift changes.
  • Providing nursing care and directing behavioral staff as necessary.
  • Assisting consumers with personal care needs, including bathing, toileting, and hygiene routines.
  • Administering medications while adhering to the Six Rights of medication administration.
  • Educating consumers and families about medications, including their uses and potential side effects, while monitoring for adverse reactions and reporting findings to the physician.
  • Effectively communicating with physicians regarding the consumer's physical and mental health status.
  • Demonstrating flexibility by working in various units based on facility needs as directed by the Charge RN or Nurse Coordinator.
  • Collaborating with the multi-disciplinary team to actively participate in the development of the interdisciplinary treatment plan.
  • Requesting and monitoring laboratory results, promptly reporting any abnormal findings to the physician.
  • Providing emergency care as required, including CPR and administering PRN medications, in accordance with AltaPointe policies.
  • Recognizing crisis situations and effectively employing de-escalation techniques.
  • Assisting with Seclusion and Restraints as per AltaPointe policies.
  • Exhibiting professional competencies and adhering to ethical standards.
  • Completing documentation in a timely and accurate manner, ensuring clarity and conciseness in consumer records.
  • Maintaining respect for consumer privacy and confidentiality while treating all individuals with dignity and compassion.
  • Actively participating in performance improvement initiatives and committee involvement as required.
  • Ensuring compliance with all AltaPointe policies and maintaining current licensure and continuing education requirements.
Physical Requirements

The role involves frequent standing, sitting, and walking, often in short bursts, with the potential for extended periods of pacing or changing positions based on consumer needs. Additional physical requirements include:

  • Occasional bending, squatting, and maintaining specific positions for brief durations.
  • Ability to lift up to 50 pounds to assist with daily living activities.
  • Capability to ambulate quickly to assist colleagues in crisis situations.
Qualifications

Must possess a current LPN license in Alabama.
