We are currently seeking a full-time Licensed Nurse to oversee and address the healthcare requirements of individuals within our Residential Services Program.

Work Schedule: Monday to Friday, day shift with flexibility and on-call support availability, compensated with an on-call stipend.

Key Responsibilities:

  • Deliver nursing services to a designated group of residential programs, which includes:
    • Coordinating and monitoring residents' health needs, ensuring all medical requirements are met and records are maintained accurately.
    • Attending medical appointments as necessary.
    • Overseeing the proper storage and management of medications across the residential programs.
    • Administering medications to individuals who are unable to self-medicate.
    • Actively participating in the development and evaluation of Individualized Service Plans (ISP).
    • Providing training and guidance to residential staff on medication administration, health monitoring, safety protocols, nutrition, and specific medical conditions.
    • Assisting residents in developing essential skills related to personal care and health management.
    • Supporting residents in achieving their individualized health outcomes.
  • Report health concerns to the Nursing Coordinator.
  • Ensure ongoing medication administration certification for staff by scheduling annual recertification and retraining following any medication errors.
  • Develop and implement training programs for staff, including specialized training for specific resident needs (e.g., urostomy care, tracheostomy maintenance, feeding tube management).
  • Assist the Nursing Coordinator in formulating and executing healthcare policies.
  • Participate in Interdisciplinary Treatment Team meetings.
  • Provide counseling to participants and staff on health maintenance and offer training as needed.
  • Act as a liaison to community resources such as physicians, laboratories, and day programs.
  • Complete all required training programs successfully.
  • Adhere to the agency's Code of Legal and Ethical Conduct.
  • Follow all organizational policies and procedures.
  • Perform other relevant job-related tasks as assigned by the immediate supervisor.

Qualifications:

  • Must hold a valid Registered Nurse license from the NYS Department of Education.
  • Preferred experience working with individuals with developmental disabilities.
  • Proficient computer skills are required.
  • Effective verbal and written communication skills in English for regulatory documentation.
  • Ability to stand and walk for extended periods.
  • Capability to assist individuals in wheelchairs and perform physical tasks necessary for patient care.
  • Valid NYS driver's license acceptable to our insurance provider and ability to travel as required.
