Job Title: Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a Physical Therapist, you will play a vital role in providing high-quality care to our patients in their homes.

Job Summary:

The successful candidate will be responsible for providing comprehensive skilled services as ordered by the attending physician. This includes observing, recording, and reporting patient reactions, changes, and progress. You will also be responsible for developing and implementing individualized plans of care, making adjustments as needed.

Key Responsibilities:
  • Provide skilled physical therapy services as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress.
  • Develop and implement individualized plans of care, making adjustments as needed.
  • Teach patients and their families on preventative measures and good decision-making.
  • Supervise and oversee therapy services provided by PTAs and nurse assistants.
  • Communicate effectively with patients, families, physicians, and coworkers.
Requirements:
  • Current license as a Physical Therapist in the state of employment.
  • Valid driver's license and auto insurance.
  • Ability to meet the physical demands of the job.
