Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Evaluate patients and develop individualized plans of care.
  • Implement and update plans of care as needed.
  • Provide education to patients and their families on proper use of equipment and techniques.
  • Communicate with patients, families, and healthcare team members to ensure seamless care.
  • Document all aspects of patient care and treatment.
Requirements:
  • Current license to practice as a Physical Therapist in the state of employment.
  • Minimum of one year of experience as a Physical Therapist in an acute setting.
  • Home care experience is desired.
  • Ability to effectively communicate and create positive impressions with patients, families, and healthcare team members.
  • Knowledgeable on therapy best practices.
  • Ability to make appropriate physical therapy judgments.
