Job Summary:The Occupational Therapist will be responsible for assessing patients' needs, developing individualized plans of care, and implementing interventions to improve their functional abilities. You will work collaboratively with patients, families, and healthcare teams to ensure seamless care delivery.
Key Responsibilities:- Conduct comprehensive assessments of patients' physical, cognitive, and emotional abilities
- Develop and implement individualized plans of care to address patients' needs and goals
- Provide occupational therapy interventions, including exercises, activities, and education, to improve patients' functional abilities
- Collaborate with patients, families, and healthcare teams to ensure effective care delivery
- Document patient progress and outcomes in accordance with agency policies and regulations
- Current licensure as an Occupational Therapist in the state of employment
- Minimum one year of experience as an Occupational Therapist in an acute care setting, with home care experience preferred
- Valid driver's license and auto insurance
- Ability to meet the physical demands of the job
