As a key member of our team, you will develop and implement tailored treatment plans to restore mobility, improve strength, and enhance overall quality of life.

Key Responsibilities:

  • Conduct thorough evaluations and create individualized treatment plans.
  • Implement therapeutic exercises and interventions to facilitate recovery.
  • Monitor patient progress and modify treatment plans as needed.
  • Educate patients and their families on rehabilitation exercises and equipment usage.
  • Collaborate with other healthcare providers to ensure optimal patient care.
  • Document patient care and progress in compliance with regulatory standards.

Requirements:

  • Current and valid Physical Therapist license in the state of practice.
  • Degree in Physical Therapy from an accredited program.
  • At least 12 months of Home Health experience within the previous 3 years.
  • Strong communication and interpersonal skills.
  • Ability to work independently and manage a caseload.
  • Basic life support (BLS) certification is required.
