Job Description

We are seeking a compassionate and dedicated Registered Nurse to join our team as a Clinical Care Specialist in Hospice Services.

About Us

Suncrest Hospice is committed to providing exceptional care and service to our patients. We strive to change the expectation of hospice care by allocating resources to increase staff-to-patient ratios, resulting in more clinical visits while reducing clinician caseloads.

Estimated Salary: $75,000 - $90,000 per year

This role offers a competitive compensation package, including a base salary range of $75,000 - $90,000 per year, depending on experience. Benefits include medical, dental, and vision insurance, a 401(k) plan with employer match, and a culture that values and appreciates its team members.

Responsibilities
  • Provide care and services for a group of hospice patients, visiting patients in their homes, assisted living facilities, or hospitals.
  • Complete hospice admissions and patient visits as needed, utilizing strong clinical and communication skills.
  • Answer calls from patients, families, and customers, addressing their needs and concerns.
  • Make nursing visits for symptom management, deaths, and admissions as required.
Requirements
  • CURRENT Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with at least one year of experience under license.
  • Admissions and On-call experience preferred.
  • Ability to work well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Strong time management skills to manage visits, documentation, meetings, and urgent needs.
About the Role

This Clinical Care Specialist will be responsible for working independently and on your own from a day-to-day basis, managing multiple activities, including phone, email, and written communication, meetings with healthcare professionals, and an understanding of hospice rules and regulations.

You will need to drive with reliable transportation, a valid driver's license, and auto insurance, to travel across the service area. This role requires a passion for hospice care and a commitment to being a great advocate for both hospice services and Suncrest Hospice.
