As a key clinical resource, you will support healthcare providers and staff in delivering optimal patient care.

Your responsibilities will include contributing to antimicrobial stewardship initiatives, along with other essential duties integral to our clinical pharmacy program.

This is a full-time position (40 hours per week, 8-hour shifts) primarily during the day, with some evening shifts and weekend/holiday commitments; typically every third weekend and 2-3 holiday shifts annually.

Qualifications:
  • BS degree required; Pharm.D. preferred
  • Completion of an ASHP-Accredited PGY1 Pharmacy Practice Residency is mandatory
  • ASHP-Accredited PGY2 Specialty Residency in Critical Care or equivalent experience (3 years in critical care) is preferred
  • Critical Care Board Certification is advantageous
  • Current licensure (or eligibility for reciprocity) in the State of Maryland
Key Responsibilities:
  • Participate in interdisciplinary rounds to provide comprehensive medication management for assigned patients
  • Engage with patients and their families to facilitate informed decisions regarding pharmacotherapy
  • Deliver accurate, timely, and cost-effective drug information to healthcare providers
  • Provide education on drug therapy to designated patient populations
  • Conduct pharmacokinetic monitoring and therapeutic adjustments as necessary
  • Review patient medication histories to ensure continuity of care during acute illnesses
  • Evaluate drug therapy for appropriateness, monitoring for effectiveness and adverse drug events (ADEs)
  • Educate patients and caregivers on medication therapies during and post-acute care
  • Collaborate with dietitians to optimize nutritional regimens as needed
  • Document clinical activities related to disease management and pharmacotherapy
  • Act as a liaison between the pharmacy department and the healthcare team to ensure adherence to drug-related policies
  • Evaluate specific medication therapies in line with departmental guidelines
  • Verify medication orders in the pharmacy database for assigned patient care areas
  • Participate in the resolution of medication-related issues on the unit
  • Engage in stewardship activities targeting anti-infectives and high-risk medications
  • Contribute to reporting ADEs to institutional committees and national programs
  • Collaborate with medical staff and administration to prepare for accreditation and address identified deficiencies
