Job Summary:
We are seeking a qualified Registered Nurse (RN) Case Manager to join our team at Harbor Healthcare System. As a RN Case Manager, you will play a vital role in coordinating patient care and working collaboratively with our interdisciplinary team.
Key Responsibilities:
- Admit and coordinate patient care, ensuring seamless transitions and effective communication with healthcare providers.
- Conduct thorough assessments, develop and implement individualized care plans, and monitor patient progress.
- Collaborate with patients, families, and healthcare teams to ensure comprehensive care and support.
- Manage patient records, maintain accurate documentation, and ensure compliance with regulatory requirements.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices.
Requirements:
- Licensed RN in the State of Louisiana - required
- 1+ years' experience as a RN - preferred
- Experience with Hospice or Home Health - preferred
- Effective written and verbal communication
- Ability to travel, must have reliable transportation
- Clean background, drug screen, and driving record
Benefits:

Work Environment:
- 8:00 AM - 4:30 PM, Monday - Friday
