Job Title: Licensed Practical Nurse

Macon Rehabilitation and Healthcare Center is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team.

Job Summary:

The Licensed Practical Nurse (LPN) will provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants. The LPN will work closely with the interdisciplinary team to ensure the highest level of quality care is provided to our residents.

Responsibilities:
  • Provide direct nursing care to residents, including administering medications, performing treatments, and providing wound care.
  • Supervise nursing assistants and ensure they are following established policies and procedures.
  • Conduct regular rounds to assess resident needs and provide care as needed.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Document resident care and progress in the medical record.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
  • Perform other duties as assigned by the Nurse Supervisor or Administrator.
Requirements:
  • Current state license as a Licensed Practical Nurse (LPN).
  • Excellent technical, assessment, and documentation skills.
  • Good leadership qualities and ability to work effectively in a team environment.
  • Organized and detailed in work performance.
  • Good communication skills and ability to work effectively with residents, families, and healthcare team members.
  • Ability to maintain confidentiality and handle sensitive information.
  • Ability to work in a fast-paced environment and adapt to changing situations.
Working Conditions:

The Licensed Practical Nurse (LPN) will work in a skilled nursing facility setting, providing care to residents with various medical conditions. The LPN will be required to stand, walk, and move frequently throughout the day, and may be exposed to infectious diseases and hazardous materials.
