Licensed Practical Nurse/Licensed Vocational Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse/Licensed Vocational Nurse to join our team at STGi. As a key member of our healthcare team, you will provide high-quality nursing care to our residents, ensuring their physical, emotional, and social needs are met.
Job Summary:The primary purpose of this role is to provide direct nursing care to residents, ensuring their safety and well-being. You will work closely with our interdisciplinary team to develop and implement individualized care plans, addressing the unique needs of each resident.
Responsibilities:- Provide direct nursing care to residents, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans, addressing the physical, emotional, and social needs of each resident.
- Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
- Monitor and report any changes in resident condition, ensuring timely interventions and adjustments to care plans.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices in nursing care.
- Provide education and support to residents and their families, promoting health literacy and self-advocacy.
- Maintain accurate and comprehensive records, adhering to regulatory requirements and facility policies.
- Current unencumbered LPN/LVN license in the state of Georgia.
- Minimum one year of experience as a supervisor in a hospital, nursing care facility, or related healthcare setting.
- Training in rehabilitative and restorative nursing practices.
- Ability to read, write, speak, and understand the English language.
- Knowledge and skills necessary to provide care appropriate to the age-related needs of our residents.
- Moderate physical activity, requiring handling of average-weight objects up to 25 pounds or standing and/or walking for more than four hours per day.
- Work is normally performed in a typical interior/office work environment.
- Required to respond to any medical emergency within the clinic, where a fast walk or job may be necessary to provide assistance.
