Responsibilities:
  • Provide direct patient care, including administering medications, performing vital signs, and providing routine care.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare professionals, including physicians, therapists, and case managers, to ensure comprehensive care.
  • Monitor patient vital signs and report any abnormalities to the case manager and physician.
  • Administer medications, feedings, oxygen, and ostomy care as needed, and document the results.
  • Complete routines established by other care professionals, such as physical therapy, occupational therapy, and speech therapy.
  • Ensure infection control policies are maintained at all times and report any possible communicable diseases or infections.
  • Accurately document nursing actions and communicate with family, case manager, and physicians.
  • Order supplies and medications as needed to ensure adequate amounts are on hand.
  • Monitor incidents involving individuals and ensure incident reports are completed timely and accurately.
Qualifications:
  • Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
  • Knowledge of individual service plans, including understanding of client rights and health needs.
  • Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
  • CPR certification.
  • Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
  • In-depth understanding of patient care policies and regulations.
