Responsibilities:- Provide direct patient care, including administering medications, performing vital signs, and providing routine care.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with healthcare professionals, including physicians, therapists, and case managers, to ensure comprehensive care.
- Monitor patient vital signs and report any abnormalities to the case manager and physician.
- Administer medications, feedings, oxygen, and ostomy care as needed, and document the results.
- Complete routines established by other care professionals, such as physical therapy, occupational therapy, and speech therapy.
- Ensure infection control policies are maintained at all times and report any possible communicable diseases or infections.
- Accurately document nursing actions and communicate with family, case manager, and physicians.
- Order supplies and medications as needed to ensure adequate amounts are on hand.
- Monitor incidents involving individuals and ensure incident reports are completed timely and accurately.
- Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
- Knowledge of individual service plans, including understanding of client rights and health needs.
- Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
- CPR certification.
- Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
- In-depth understanding of patient care policies and regulations.
