Job Description

We are seeking a compassionate and skilled Care Coordinator RN to join our team at Chapters Health System. As a key member of our Interdisciplinary Group, you will play a vital role in assessing and identifying patient/family needs, coordinating the Plan of Care, and providing palliative and supportive care.

Salary and Benefits

The estimated salary for this position is $60,000 - $80,000 per year, depending on experience. In addition to a competitive salary, we offer a comprehensive benefits package, including sign-on bonus, benefits from day 1, and opportunities for career growth and development.

Job Responsibilities

As a Care Coordinator RN, your responsibilities will include:

  • Assessing and identifying patient/family needs
  • Coordinating the Plan of Care with the Interdisciplinary Group
  • Providing palliative and supportive care to patients and families
  • Communicating effectively with healthcare professionals, patients, and families
  • Supervising patient care given by other nursing team members
Required Skills and Qualifications

To be successful in this role, you will need:

  • A current license as RN in the state where you will be working
  • Minimum one (1) year of in-patient nursing or hospice experience for external candidates; for internal applicants, minimum of 6-months of LPN experience with hospice/home health within an affiliate
  • Previous experience working with an EMR/EHR system
  • Mobile Driver - Valid driver's license and automobile insurance per Company policy
  • Ability to use equipment with visual and auditory mechanisms
  • Ability to effectively communicate in English (verbal and written)
